Why does 我在吃饭 mean "I'm eating right now," but 我在家吃饭 just means "I eat at home"?

Same 在, two different jobs, and the next word decides which one.

Chinese 在 does two unrelated jobs, and the word right after it picks the job. If a place comes next, 在 means "at" and simply tells you where something happens. If an action comes next, 在 means "in the middle of doing it right now." English fakes both with one word, "at," which is why learners get fooled. So 我在家吃饭 is a plain fact about where you eat. 我在吃饭 is a live status update. Can I stack them and say 我正在在家吃饭 for "I'm eating at home right now"?

No. Two 在s bang into each other, so split them up instead.

Chinese hates two 在s smashed together, so 正在在家吃饭 sounds like a stutter. Natives solve it in three easy ways. Drop the 在 from the place and say 我正在家吃饭. Add 现在 and say 我现在在家吃饭. Or stay casual with 我在家吃饭呢. Pick one frame, never both.

What's the difference between 我在吃饭 and 我正在吃饭, or are they the same thing?

Can I put it at the end and say 我吃饭在 for "I'm eating right now"?

No. 在 and 正在 always sit before the verb, never after it.

English throws "right now" wherever it wants, including the tail end of a sentence. Chinese won't allow that. 在 and 正在 are glued to the front of the verb, so the shape is Subject + 在/正在 + Verb, full stop. Push them behind the verb and it sounds broken, like saying "I eating am" in English.
